Rema distinguishes between chanann of meat & milk and other prohibitions (shar issurim). The Rema is stringent to follow Rashi’s opinion who rules that a semi-submerged piece of meat that receives a drop of milk becomes prohibited in its entirety - chanann (if not 60 in that piece). But Rema is lenient if that drop was shar issurim. The Shach (92:12) also says chanann of shar issurim is only rabbinical. However, Rebbi Akiva Eiger says according to Rabbaynu Tam chanann bishar issurrim is a Torah level prohibition. Tosfos (Chullin 100..) challenges Rabbaynu Tam from hechshur kli Midian (Numbers 31:23). At that time, there were no rabbinic prohibitions.
